Probably a Saint.
As the flag has the coat of arms of Siena, it may be a saint associated with Siena, perhaps San Martino.


He is seated forward in a legs forward style, but facing around to his left rear.  Is there something attacking the back leg of the horse?

Thanks Druzhina. That really fits, if you are referring to St.Martin of Tours. He had a famous story about him of how he was riding on his horse and met a beggar, and the saint (who was a soldier at the time) cut his cloak in half with his sword and gave one half to the beggar and kept the other half for himself.

This seems to be often represented as St.Martin turning backwards on his horse to face the beggar who stands at the horse's rear -

Yes, the left hand may be bunching a red cloak, a beige blob may be the face of a kneeling(?) beggar.

St. Martin is also often depicted in Italian art, e.g. St. Martin Renounces His Weapons by Simone Martini, 1312-17
