Should pronouns (He, Him, His) be capitalized when referring to the Lord God and Christ Jesus?     
I have seen some Catholic bibles where the religious pronouns are not capitalized.

Does it apply to 'Her', too - as in 'Mary Mother of God'?

Or pre-monotheistic G/gods, or passé G/gods of defunct religions?

This type of capitalization is known as reverential capitalization. However this isn't restricted to pronouns alone, with some extending this to any term which refers to the deity. What I can gather is that these types of capitalization has been in decline for many years.

In all of the earliest copies of the Greek New Testament, the text is written in all capital letters.
