Can you list and countries that have transitioned peacefully from a dictatorship to democracy in modern times? Please include a brief narrative about how it happened.

The Elections of 2000. The PRI was ousted out of the presidency after 71 years.
Now, we will take PAN out of the office

Mexico in 2000 is indeed a good example.

Other examples:
Spain in 1975. Franco died, appointed Juan Carlos as his sucessor, and democracy was reintroduced.

East Germany (and some other Eastern European Countries as well) 1989. After the SED government collapsed the first democratic elections were held and one year later Germany was re-united

South Africa 1994 and Chile 1989 are other examples

I still really can't believe , how relatively smooth and bloodless the transition from the Apartheid regime to majority rule in South Africa went.
It's been nothing short of a miracle.

How about the US in 2004, the leader didn't change but at least this time round he was democratically elected.

I have my doubts about that. There were a number of tactics that were not technically illegal, but nevertheless disinfranchised many voters.

Perhaps modern America is a better example of a modern nation transisting peacefully from democracy to dictatorship?
