About Persia, I think Fathalishah Qajar (reigned 1797 - 1834) was the last king who led troops in battles, but the last major battle was the Battle of Krtsanisi: http://en.wikipedia.org/wiki/Battle_of_Krtsanisi in 1795 between Persian army, led by Agha Mohammad Khan, the founder of Qajar Empire, and Georgians at the place of Krtsanisi, near Tbilisi, the capital of Georgia.

During WWI, Germany was an empire composed of a variety of states including kingdoms.  Anyways, several German crown princes, Grand Dukes etc. followed the medieval custom and served as army commanders.  One of those was Crown Prince Weilhelm who led the German 5th Army at Verdun.  In addition, Tsar Nicholas, though not an army commander per se, was heavily involved in the day to day leadership  and military planning activities of Russian armies in the field.
